29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a. On or about 11/16/2017, at the job site - employees conducting steel erection tasks such as installing metal roofing and welding were exposed to about 15 feet 3 inch from the top of the beams to the ground, in that, fall protection was not used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. On or about 11/16/2017, at the job site - the rails of the ladder used to climb up or down the steel beams did not extend at least 3 foot above the upper landing surface, exposing the employees to a 15 feet 3 inch fall hazard.
