29 CFR 1910.147(c)(1): Energy Control Program. The employer shall establish a program consisting of energy control procedures, employee training, and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    On or about November 16, 2017, the employer did not develop and document a Hazardous Energy Control Program to include topics such as but not limited to a written program and periodic inspections for when employees perform servicing, maintenance, and blade changes on equipment such as but not limited to: propane forklift, three-head pallet ripper, hydraulic single head ripper, chop saw, and stringer press exposing employees to accidental/inadvertent start up of equipment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    During the month of November and thereafter, the employer did not provide effective means of machine guarding such as two hand controls or other equivalent guarding to the Swinger Press machine. The employee operated machine with one hand and the other hand was not occupied to protect employee from caught in hazards as machine cycled.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination shall be protected from accidental contact or breakage by a suitable fixture or lamp holder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded.    On or about November 16, 2017, fluorescent lights in the break room and storage area were not protected from accidental contact or breakage exposing employees to electrical shock hazards.

29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer.  On or about November 16, 2017 employees wearing The Safety Zone dust mask on a voluntary basis were not provided with the information in Appendix D.
