29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:  Instance 1) For the employees working on the drilling rig on or about November 15, 2017 and times prior there to, the employer did not ensure employees working on the rig floor by the doghouse were protected from falling.  This exposes employees to the hazard of falling from the rig floor.  Instance 2) For the employees working around the pipe rack and the mud pit on or about November 15, 2017 and times prior there to, the employer did not provide fall protection along the walking path at the edge of the mud pit.  This exposes employees to the hazard of falling into the mud pit.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:  1) For the employees working on or around the mud mixing tanks on or about November 15, 2017 and times prior there to, the employer did not ensure the wiring to the lighting that was approved for the hazardous classified location were installed in a safe manner.  This exposes employees to explosion or fire hazards.  2) For the employees working on the mud mixing tanks on or about November 15, 2017 and times prior there to, the employer did not ensure splices in flexible electric cords to the mud logger gas detector was safe for the hazardous classified location by the shale shaker.  This exposes employees to the hazard of explosion or fire.
