29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about 11/17/17 on the north side of the metal recycling area; employees who operated this baler were exposed to a crushing injury when the safety switch on the gate, preventing access to the ram platen, did not deactivate the ram when the gate was open and the ram was ascending. b) On or about 11/17/17 on the west side of the metal recycling area; employees who operated this baler were exposed to a crushing injury when the safety switch on the gate, preventing access to the ram platen, did not deactivate the descending ram when the gate was opened ABATEMENT CERTIFICATION REQUIRED
