Water from the north side boiler was left to pool on the floor on the east side of the boiler room.    b. The utility sink drain on the north wall of the boiler room was damaged and drained directly onto the floor and was not cleaned.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  On or about November 16, 2017 employees were exposed to electric shock and fire hazards and the employer did not ensure that the live electrical fixtures were effectively closed:  a. The east junction box near the west entrance to the boiler room provided 220 volt power to silo controls and the cover was not completely attached and wires were hanging outside the box.  b. The west junction box near the west entrance to the boiler room provided 220 volt power to silo controls and the cover was not completely attached and wires were hanging outside the box.  c. The 3 phase, 3 pole junction box on the south wall in the boiler room that had powered blowers was missing the front cover and live conductors were exposed.  d. The single phase junction box on the west wall in the boiler room that powered outside wires was missing the front cover and live conductors were exposed.  e. The polyphase junction box on the west wall in the boiler room that powered the heater in the room was missing the front cover and live conductors were exposed.  f. The polyphase junction box on the west wall in the boiler room that powered the silo feeder controls was missing the front cover and live conductors were exposed.  g. The electric panel on the south wall near the air compressor in the boiler room was missing the front cover and live conductors were exposed.  h. The junction box on the south wall in the boiler room below and connected to multiple electric panels was missing the front cover and live electrical wires were exposed.  i. The junction box on the south wall above the heater power switch in the truck repair shop that controls the heaters did not have the cover secured and live electrical wires were exposed.
