29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. On or about November 17, 2017, employees were exposed to ingoing nip points created by the conveyor and drive wheel while checking the conveyor for jams and cleaning out entangled material.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes. On or about November 17, 2017, employees were exposed to an unguarded belt and pulley on the conveyor drive motor located on the catwalk at the end of the conveyor.

(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: guardrail systems; safety net systems; or personal fall protection systems such as personal fall arrest, travel restraint, or positioning systems: On or about November 17, 2017, employees pushing material into open top trailers on the loading ramp are exposed to fall hazards of approximately 7 feet between the ramp and trailer without any form of fall protection. The Landfill Reduction and Recycling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 1162872, citation number 1, item number 2 and was affirmed as a final order on November 2, 2016, with respect to a workplace located at 7102 Hwy 12 - Transfer Station Madison, WI 53718.

The employer must ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening is less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level is protected from falling by the use of guardrail systems; safety net systems; travel restraint systems; or personal fall arrest systems: On or about November 17, 2017, employees working on the sorting line located on the second story were exposed to fall hazards of approximately 16 feet into chutes with guardrails measured at 33 inches while sorting and throwing material into the chute. The Landfill Reduction and Recycling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(a)(3), which was contained in OSHA inspection number 1162872, citation number 1, item number 1 and was affirmed as a final order on November 2, 2016, with respect to a workplace located at 7102 Hwy 12 - Transfer Station Madison, WI 53718.
