29 CFR 1926.51(f): Adequate washing facilities were not provided for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants might have been harmful to the employees:  a.    On or about November 16, 2017, employees using roofing cement adhesives were not provided adequate handwashing facilities to remove the roofing cement or the gasoline they used as a solvent from their hands prior to eating their lunch, and they were exposed to the hazards of skin irritation and ingestion of the chemicals.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.    On or about November 16, 2017, on the 5:12 pitch roof, 5 roofing employees were exposed to 8 feet to 12 feet fall hazards from the eaves, gables, and skylight, in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  a.   On or about November 16, 2017, against the front of the house, the aluminum extension roof access ladder was set up at a 61-degree angle, shallower than required by the standard to prevent slippage of the ladder.
