29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) Frontier Airlines at 26360 E 103rd Ave. Denver, Co 80249: On or before 11/9/17 the employer did not protect employees from injury in that unapproved Vestil fork extenders were used on the Nissan 80 forklift (bumper#10004). The fork extenders are utilized to move larger crates of parts weighing up to 2500 lbs. This condition exposed employees to an unstable forklift.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition: (a) Frontier Airlines at 26360 E 103rd Ave. Denver, Co 80249: On or before 11/9/17 the employer did not ensure the data plates on the Nissan 50 and Nissan 80 fork trucks were legible. This condition prevents employees from knowing the safe load limits for each truck.
