(serial no.: 0133339-2008B) battery charger was not protected by pipe bollards, elevation, or other effective means of guarding, to protect the charger from being struck by power industrial trucks delivering batteries for charging. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Warehouse, Zone A - On or about November 16, 2017, a Daewoo B20T (#8) power industrial truck, used to elevate workers to the upper storage racks in a warehouse by means of a platform attachment, was not marked to indicate the weight of the truck and the attachment combination when at its maximum elevation with the load laterally centered. No abatement certification or documentation required for this item.
