29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) On or about November 16, 2017 and at times prior, two employees were exposed to a catapult hazard and subsequent fall of approximately thirteen feet to the ground below while using the S-65 Genie aerial lift without fall protection.
