At the worksite - On or about November 16, 2017, an employee operated a pneumatic nail gun without the use of eye protection, thus exposing the employee to a struck-by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a). At the worksite - On or about and times prior to November 16, 2017, employees were working on the roof conducting activities to include, but not limited to removing and installing roof shingles without the use of a fall protection system in place, thus exposing employees to a fall hazard of 23 feet.
