On or about November 16, 2017, the employer did not complete a personal protective equipment hazard assessment, and did not verify the assessment through a written certification. On or about November 16, 2017, the employer did not develop or implement a hazard communication program to ensure employee protection while using Keratin Complex products that contain timonacic, which can produce formaldehyde while used in the hair smoothing process.     Abatement certification is required of this item in accordance with 29 CFR 1903.19(c).

On or about November 16, 2017, the employer did not maintain safety data sheets for chemicals including, but not limited to Keratin Complex products that contain timonacic, which can produce formaldehyde while used in the hair smoothing process.   There is no abatement certification or documentation required for this item.
