29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user.    On or about November 20, 2017, employees were exposed to respiratory stressors associated with being allowed to voluntarily wear Miller LPR - 100 half-face tight-fitting respirators while welding on steel such as, but not limited to aluminum, mild, and stainless steel.The employer did not establish and implement elements of a written respiratory program to ensure employees were medically able to wear respirators and that respirators were cleaned, stored, and properly maintained.        There is no abatement certification or documentation required for this item.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:   On or about November 20, 2017, employees were exposed to respiratory stressors associated with being allowed to voluntarily wear Miller LPR - 100 half-face tight-fitting respirators while welding on steel such as, but not limited to aluminum, mild, and stainless steel. The employer did not provide medical evaluations to employees prior to the voluntary respirator use.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.134(h)(2)(i): All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve.  On or about November 20, 2017, employees were exposed to respiratory stressors associated with being allowed to voluntarily wear Miller LPR - 100 half-face tight-fitting respirators while welding on steel such as, but not limited to aluminum, mild, and stainless steel. The employer did not ensure the respirators were being properly stored from damage, contamination, and exposure to chemicals while not in use.      There is no abatement certification or documentation required for this item.

29 CFR 1910.252(b)(2)(iii): Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles.      On November 20, 2017, employees working adjacent to the welding area were potentially exposed to health hazards such as arc eye and welder's flash caused by welding rays. The employer did not protect adjacent employees by use of welding shields/screens or appropriate eye goggles.       Abatement certificationn is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1026(l)(1)(iii): The employer had not included chromium (VI) in the hazard communication program, established to comply with the Hazard Communication Standard, 29 CFR 1910.1200, ensured that each employee had access to labels on containers of chromium (VI) and to safety data sheets, and was trained in accordance with the requirements of the Hazard Communication Standard and 29 CFR 1910.1026(l)(2).    On or about November 20, 2017, employees were being exposed to welding fumes while welding on stainless steel and the employer did not train employees on the physical and health hazards of hexavalent chromium (chromium VI).
