On or about November 13, 2017 the employer allowed employees to use a Dewalt 7-1/4" Circular Saw (DWE575) in front of the building to cut wood block/planks for door framing and molding for inside the building without ensuring they utilized appropriate eye protection.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about November 13, 2017, the employer allowed the employee to operate/drive and work in the lift basket without a harness on nor a lanyard attached on the South side of the building. The employees was using the aerial to work on the eaves of the building and to access the roof top.
