29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.    At the new residential construction site, employees were walking and working from the roof and not protected from falling more than 18 feet to the lower level by use of guardrail or personal fall arrest system.       Eduardo Perez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1198825, Citation Number 1, Item number 1 and was affirmed as a final order on February 22, 2017, with a final abatement date of 03/08/2017, with respect to a workplace located at 301 T.C. Lupton Road, Sunnyvale, Texas 75182.  Eduardo Perez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11) , which was contained in OSHA inspection number 1096952, citation number 1, item number 1 and was affirmed as a final order on 11/12/2015, with a final abatement date 11/27/2015, with respect to a workplace located at4215 Cascade Sky Drive, Arlington, TX 76005.

29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At the new residential construction site, employees used ladders that were not extended at least 3 feet above the upper landing to access the working area exposed to falling more than 18-feet from the top plates.  Eduardo Perez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1096952, citation number1, item number 2 and was affirmed as a final order on 11/12/2015, with a final abatement date of 11/27/2015 with respect to a workplace located at 4215 Cascade Sky Drive Arlington, TX 76005.   Eduardo Perez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1061427, citation number 1, item number 1 and was affirmed as a final order on 6/16/2015, with a final abatement date of 06/24/2015, with respect to a workplace located at 1816 Crystal Cove Wylie, TX 75098.
