Oregon, OH: The employer failed to ensure the welded frame scaffold system from which employees were working during plastering activities was secured at or below the 4:1 height to width ratio. The 40 ft. high by 5 ft. wide scaffold system did not maintain a securing means to the adjacent structure until the height of 32 ft. Employees were exposed to falls, in that adequate stability of the system was not provided while performing work tasks from the scaffold.
