29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. Drill Site: On or about November 14, 2017; employees were exposed to electrical shock hazards because the electrical circuit breaker boxes at the end of the trailers had unprotected openings in the following instances. a) OPR trailer #11797, drilling crew trailer #1. b) OPR trailer #11798, drilling crew trailer #2.

29 CFR 1910.303(b)(7)(iv): There was damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: Drill Site: On or about November 14, 2017, the outside light fixture next to the door on OPR trailer #11797 (crew trailer #1) was missing the weather proof cover. This condition exposed the employees to electrical shock hazards.
