On or before December 15, 2017, on the third floor mill, the Buhler-Miag 4 Midds #1 roller mill/roll stand lacked a strong enough grate that would prohibit the employees from gaining access to the inside of the stand. On or about November 10, 2017, an employee sustained an injury when reaching through the existing grate and into the stand.
