29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.     a. On or about November 10, 2017, on the 3rd floor mill, the Buhler-Miag 4 Midds #1 roller mill/roll stand was not shut down in an orderly fashion using the procedures established for the equipment which resulted in multiple finger amputations for an employee reaching into the working rolls (grinding rolls) to remove an object.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).      a. On or about November 10, 2017, on the 3rd floor mill, the Buhler-Miag 4 Midds #1 roller mill/roll stand was not isolated from its electrical energy source which resulted in multiple finger amputations for an employee reaching into the working rolls (grinding rolls) to remove an object.

Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.     a. On or about November 10, 2017, on the 3rd floor mill, verification that isolation and deenergization of the Buhler-Miag 4 Midds #1 roller mill/roll stand was not accomplished prior to an employee reaching into the working rolls (grinding rolls) to remove an object which resulted in multiple finger amputations.

OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.    a. On or before December 15, 2017, on the third floor mill, the  Buhler-Miag 4 Midds #1 roller mill/roll stand lacked a strong enough grate that would prohibit the employees from gaining access to the inside of the stand.  On or about November 10, 2017, an employee sustained an injury when reaching through the existing grate and into the stand.
