They were using a rough terrain forklift that had a big windshield crack in the line of sight. This exposed employees to probable struck-by hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Rig Site #650: On or about November 15, 2017, employees were working where there were two instances of listed or labeled electrical equipment: 1. They were using a fifty foot 12AWG three way extension cord to connect to two fifty foot 10AWG extension cords. 2. They were also using a power tap to plug in a microwave and a coffee pot. These issues exposed employees to fire and/or burn hazards.
