29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. In the Production Department, employees performing testing protocols on the Queue Drive Motor units were exposed to caught-in and amputation hazards were the machine was not guarded to prevent employee exposure.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. The employer failed to report to OSHA a work-related hospitalization, a hospitalization occurred on 10 November 2017 and was not reported to OSHA until 13 November 2017.
