29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.       At the new residential construction site, employees were walking and working from the roof and not protected from falling more than 18 feet to the lower level by use of guardrail or personal fall arrest system.

29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      At the new residential construction site, employees used ladders that were not extended at least 3 feet above the upper landing surface to access the working area exposing them to a fall hazard more than 18-feet.
