29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: A. Guardrail systems; B. Safety net systems or C. Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: Line #8 Freezer Bar. On or about November 13 through November 22, 2017 employees were exposed to falls from heights above 4 feet from the following elevated work platforms with inadequate guardrail systems: a) Palletizer #2 - Side of platform, unprotected opening approx. 31" high x 25" wide. b) Palletizer #3 - Corner of platform to the left of operator position, unprotected opening approx. 22" high x 24" wide. c) Palletizer #3 - Side of platform behind operator position, unprotected opening approx. 20" high x 24" wide. d) Palletizer #3 - Platform. Open-sided corner of platform, next to access stairs. e) Palletizer #2 - Upper work platform, open-sided, near sheets applicator. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Line #8 Freezer Bar, Palletizer #2. On or about November 13 through November 22, 2017 employees were exposed to contact with unguarded rotating parts on the in-feed conveyor. A light curtain safeguarded a portion of the conveyor line, but was not properly applied to prevent employees from contacting rotating parts on all sections of the conveyor when lifting, turning or repositioning packages on the conveyor. Employees were exposed to amputation and laceration hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
