29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. On or about November 21, 2017, Units 7 thru 11, Atlanta, Georgia, employees erecting the pump jack scaffolding were exposed to an electrical hazards when a section of the scaffold contacted the 11 kV power lines.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about November 21, 2017, Units 7 thru 11, Atlanta, Georgia, two employees setting up a pump jack scaffold approximately 35-foot tall contacted an overhead power line. The employer did not train the employees how to recognize and minimize the hazards of working in the proximity of overhead power lines.
