29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a)  At ground level near the All Points Transport Warehouse, on or about November 27, 2017, the employer did not enforce the use of protective helmets for employees walking and working below employees working overhead, exposing them to struck-by hazard.

29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:    (a)  While working from an aerial lift at the All Points Transportation warehouse, on or about November 27, 2017, an employee climbed the aerial lift railing to work and access the roof of the structure, resulting in an approximately 16-foot fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.  (a)  While working from an aerial lift at the All Points Transportation warehouse, on or about November 27, 2017, fall protection was not in use by employees working to perform roofing material installation, resulting in an approximately 16-foot fall hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a)  On the rooftop of the All Points Transportation warehouse, on or about November 27, 2017, the employer did not ensure the use of fall protection for employees performing roofing material removal and installation on a 3:12 pitch roof, exposing employees to an approximately 16-foot fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  At the roof line of the All Points Transport warehouse, on or about November 27, 2017, the employer allowed employees to access the roof by a ladder that was extended approximately 2.5 feet above the roof line, exposing employees to an approximately 16-foot fall hazard.
