29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: a) Door Shop: On or about November 28, 2017, employees were exposed to the unexpected release of energy while conducting maintenance, replacing router bits, clearing jams, and adjusting equipment such as but not limited to the energized Norfield Magnum Door Machine, serial number DJ1335M, and the SCMI Molder Machine while using emergency stop buttons and interlocks for energy isolation.
