29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes and fittings were not effectively closed. On or about November 22, 2017, and in times prior thereto, an one inch opening was located in the bottom of a breaker panel used to provide temporary power at the construction site, exposing workers to the hazards of electrical shock.

29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures: On or about November 27, 2017, and in time prior thereto, the fitting protecting the power cord from the sharp edges of the electrical panel box was dislocated creating an electrical hazard to construction workers using the panel, located at the constructions site.
