29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. At the jobsite, employees were not wearing hardhats while working below other employees that were installing cornice trimming to a residential house, exposing employees to struck-by hazards from falling objects.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. At the jobsite, employees working from the upper levels of a wooden type scaffold that was not equipped with guardrails were exposed to the hazard of falling more than 12ft. to the ground below.
