29 CFR 1910.29(b)(1): (1) The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11). (a) Employees working on the mezzanine level in the Line 4 Packing Department, were exposed to a 10 foot, 11 inch fall as the result of the guardrail system not being installed directly above the walkway.

29 CFR 1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access: (a) The exit route for employees working on the mezzanine level in the Line 4 Packing Department, was partially blocked by a 24 inch wide barrel, thereby allowing employees only 8 inches to pass between the barrel and the railing.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available: (a) On or about November 17, 2017 an employee who fell from the Line #4 Mezzanine, was not provided with medical attention in a timely manner, in that the injured employee had to wait 16 minutes, 47 seconds for medical help arrive.
