29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    (a)  Horvath Roofing, Inc. on or about November 30, 2017, at the address of 2624 Milan Rd. Sandusky, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Sandusky, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type structure.  Employees operating in this condition were exposed up to a 10 ft. fall to the ground below.

Sandusky, OH:  The employer did not develop or maintain a written record of fall protection training certification for all employees who might be exposed to fall hazards. The employer failed to evaluate the adequacy and certify fall protection training that employees had participated through the employer.
