29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about 11/30/17 on the roll grader system; employees used a compressed air wand to clean in and around the discharge of # 5 hopper and were exposed to an unprotected fall of 5.5 feet into the hopper when the top perimeter (three sides) of the floor level hopper was open.    b) On or about 11/30/17 on the roll grader system; employees used a compressed air wand to clean in and around the west side of hopper #4 and were exposed to an unprotected fall of 5.5 feet when height of the frame was between 25 - 16 inches from the work platform.    c) On or about 11/30/17 on the roll grader system; employees used a compressed air wand to clean in and around the west side of hopper #3 and were exposed to an unprotected fall of 5.5 feet when the height of the frame was between 36 - 26 inches from the work platform. 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) On or about 11/30/17 on the outside roll grader system; employees were exposed to an amputation hazard while attempting to remove debris from the rotating rollers on the roll topper when the cover over the rollers was easily removable permitting access to the rollers.      b) On or about 11/30/17 on the outside roll grader system: employees were exposed to an amputation hazard when the grease fittings on the west end of the roll topper rollers were exposed allowing access to the rotating rollers.      c) On or about 11/30/17 on the outside roll grader system; employees were exposed to a caught between hazard when the rubbers fingers on the intake end of the cleaner were not guarded on either side allowing access to these rotating parts.      d) On or about 03/29/18 on the conveyor for the roll grader system; employees were exposed to caught between hazard on the ingoing nip point of the bottom roller located at grade.      e) On or about 03/29/18 on the drag line feeding the conveyor; employees were exposed to caught on hazards when the drag line flights feeding the conveyor were not guarded at a height of approximately 6 feet from grade. 29 CFR 1910.144(a)(1)(iii): Emergency stop buttons or switches which letters or other markings appear, used for emergency stopping of machinery were not red.  a) On or about 12/01/17 at the top of the stairs opposite the roll topper; employees were exposed to a minimal  hazard when the large round shiny silver colored button in the upper left hand corner on the control panel was not labeled but  appeared to be an emergency stop button.    b) On or about 12/01/17 at the bottom of the roll topper; employees were exposed to minimal hazard when the large round dull color plastic button attached on the lower portion of the roll topper support leg was represented to be an emergency stop button and was not labeled or red in color.
