29 CFR 1910.132(d)(2):  The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:                                                                                                                  In the EMS building, the employer did not perform any hazard assessment to protect and train employees of the hazards associated with gases that necessitate the use of personal protective equipment (PPE) exposing employees to health hazards.

29 CFR 1910.134(d)(1)(iii):  The employer did not identify and evaluate the respiratory hazard(s) in the workplace; this evaluation shall include a reasonable estimate of employee exposures to respiratory hazard(s) and an identification of the contaminants chemical state and physical form. Where the employer cannot identify or reasonably estimate the employee exposure, the employer shall consider the atmosphere to the IDLH:                                                                                                                 In the EMS building, the employer did not identify and evaluate the respiratory hazards in the EMS building where employees work with Nitrogen Dioxide, Nitric Oxide, Ozone, Ammonia and Sulphur Dioxide gases exposing employees to respiratory and other health hazards.

29 CFR 1910.1200(h)(1):  Employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g. flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets:                                                                                                                 In the EMS building, the employer did not provide employees with the required information and training on gases such as Nitrogen Dioxide, Nitric Oxide, Ozone, Ammonia and Sulphur Dioxide being used daily in the EMS building exposing employees to health hazards.
