29 CFR 1910.1200(h)(3)(ii): Employees were not trained on the physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area:  TRADER JOE'S WAREHOUSE, 200 PHOENIX CROSSING, BLOOMFIELD, CT:  On or about and at times prior to November 30, 2017, employees who were operating equipment with internal combustion engines indoors were not provided training on the signs and symptoms of carbon monoxide exposure.

29 CFR 1926.55(a): Employees were exposed to material(s) at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists:  TRADER JOE'S WAREHOUSE, 200 PHOENIX CROSSING, BLOOMFIELD, CT:  On or about, and at times prior to November 30, 2017, employees pouring a concrete floor inside a freezer warehouse building that was under construction were exposed to carbon monoxide at concentrations in excess of the 8 hour Time Weighted Average (TWA) Permissible Exposure Limit (PEL) of 50 parts per million (ppm).  Employees were using approximately 12 gasoline- or diesel-powered machines to pour, smooth, and finish the concrete inside the building.  a)  One employee was transported to a local hospital via ambulance following complaint of severe headache and difficulty breathing.  The employee had been exposed to carbon monoxide at an 8-hour TWA of 100 ppm which is twice the PEL.  The exposure concentration was derived by entering the employee's carboxyhemoglobin (COHb) concentration and other site-specific data into the Coburn-Forster-Kane equation.

29 CFR 1926.55(b): Feasible administrative or engineering controls were not implemented to reduce employee exposure(s):  TRADER JOE'S WAREHOUSE, 200 PHOENIX CROSSING, BLOOMFIELD, CT:  On or about and at times prior to November 30, 2017, employees were exposed to carbon monoxide while pouring a concrete floor inside a 60,000-square foot warehouse facility.  Employees were using twelve gasoline- or diesel-powered machines (mud buggies, laser screed, ride-on trowels) inside the building.  There was limited ventilation provided by fans that were installed in man doors.  The Fire Department was called to the scene when one employee exhibited symptoms of carbon monoxide exposure.  The employee was transported to the hospital.  The Fire Department monitored for carbon monoxide and recorded measurements from 139 to 150 parts per million inside the building.
