29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a)H & R Roofing of South Dakota, Inc. at 2516 South Minnesota Avenue, Sioux Falls, SD 57105: On or about and at times prior to November 30, 2017, the employer did not ensure that each employee was protected from injury, in that employees engaged in roofing activities were exposed to falls in excess of 6 feet without the use of a fall protection system. This condition exposed employees to a fall hazard of approximately 16 feet 8 inches.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a)H & R Roofing of South Dakota, Inc at 2516 South Minnesota Avenue, Sioux Falls, SD 57105: On or about and at times prior to November 30, 2017, the employer did not ensure that each employee engaged in roofing operations on a commercial property, had been trained to recognize the hazards associated with working at elevations over 6'. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The condition exposed employees to the hazard of falling from an elevation of approximately 16 feet 8 inches.
