29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) On or about November 28, 2017, employees installing cornices to a single family residential home located at 5924 Barrique Blvd, McKinney, TX.  At least one (1) employee was working from the ground doing various work operations and did not have on a hard hat.  The employees were exposed to head injuries where overhead hazard exists.

29 CFR 1926.102(a)(1):   Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:         (a) On or about November 28, 2017, employees installing cornices to a single family residential home located at 5924 Barrique Blvd, McKinney, TX.  Employees were doing cornice work while using nail guns. Employees were not wearing safety glasses and were exposed to eye injuries.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws:    (a)  On or about November 28, 2017, employees installing cornices to a single family residential home located at 5924 Barrique Blvd, McKinney, TX.  Employees using a Jenny Air Compressor, Model K, Serial number G050416007 had a strain relief exposed them to electrical hazards.

Employees using an electrical cord that did not have a ground pin, exposing employees to an electrical hazard.

Employees using a ten (10) step job made ladder that did not reach the work platform, exposing employees  fall hazards of approximately 10-feet to the ground level.

Employees using an 8-foot, Werner ladder with the spreaders closed, employees were exposed to a fall hazard.
