In the garage area the employer did not ensure that the DeWalt table saw was being used with the guard installed as required. Employees were exposed to an amputation hazard.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided to prevent pull from being directly transmitted to joints or terminal screws: In the garage area the power cord on the DeWalt cross-cut saw no longer had the outer insulation attached and thereby lacked strain relief where it attached to the saw. Employees were exposed to a shock hazard.
