29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level:  The employer did not ensure that the employees utilizing the scaffold on the southwest corner of the residence were provided fall protection. The employees were exposed to a fall hazard in excess of ten (10) feet to the ground below.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  The employer did not ensure that employees performing roofing operations on the front and back of the residence were provided fall protection. The roof had a pitch of up to 8 in 12 and the employees were exposed to a fall hazard of approximately ten (10) feet eave to the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  The employer did not ensure that the portable ladder being used to access the roof extended more than approximately two feet above the eave. Employees were exposed to a maximum fall hazard in excess of ten (10) feet to the ground.
