29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about November 30, 2017, the employer had not established a program for the control of hazardous energies for employees operating and servicing equipment at the facility. Equipment operated and maintained at the facility includes but is not limited to presses, screw machines, cranes, and CNC machines. Hazardous energies include but are not limited to electrical, pneumatic, hydraulic, and mechanical.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about November 30, 2017, the employer failed to ensure that screw machines located in the facility were effectively guarded to protect employees from the moving machinery inside the equipment. Doors on both sides of the machines were either not interlocked or able to be opened during operation. The following screw machines were not effectively guarded at the time of inspection;  #950, #1130, #1108, #866, #807, #245, #383, #24, #23, #20, #19, #26.
