29 CFR 1926.451(f)(14):  Makeshift devices, such as but not limited to, boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:    (a)  AV Company LTD, at 3201 23rd Ave., Greeley, CO 80631:  On and before November 30, 2017, the exposing employer allowed employees to use makeshift devices, such as but not limited, to boxes and barrels, on top of scaffold platforms to increase the working level height.  Employees used 5 gallon buckets and stacks of foam pads on top of scaffold platforms to increase the working height.  This condition exposed employees to fall hazards of approximately 11 feet.
