29 CFR 1926.451(d)(3)(vii): Tiebacks were not equivalent in strength to the suspension ropes:  a.  On November 3, 2017, at the above addressed jobsite, the tiebacks used on the suspended scaffold were not the same strength as the suspension ropes. The scaffold tieback had a six (6) foot lanyard tied in the middle of the rope to extend the length of the tieback, thereby exposing the employees to hazards associated with falls greater than three (3) stories.

29 CFR 1926.451(g)(3): In addition to meeting the requirements of 1926.502(d), personal fall arrest systems used on scaffolds shall be attached by lanyard to a vertical lifeline, horizontal lifeline, or scaffold structural member. Vertical lifelines shall not be used when overhead components, such as overhead protection or additional platform levels, are part of a single-point or two-point adjustable suspension scaffold.   a.  On or about November 3, 2017, at the above addressed jobsite, employees performing tuckpointing operations from a suspended scaffold, greater than three (3) stories above the ground, did not have an anchor point for the personal fall arrest system separate for the suspended scaffold, thereby exposing the employees to falls.
