29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems: The employer is failing to protect employees from fall hazards when conducting roofing operations on commercial structures. This was most recently documented at Willow Lakes Golf Course, Offutt AFB, Nebraska on or about November 30, 2017 when six employees were observed installing shingles to a commercial structure while working greater than 6 feet above the ground without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to protect employees from fall hazards when conducting roofing operations on a commercial structure. This was most recently documented at the Willow Lakes Golf Course, in Offutt AFB, Nebraska on or about 30 November, 2017 when employees were observed using a ladder to access a roof and the ladder's side rails did not extend at least 3 feet past the upper landing surface.
