29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:    a) Jobsite: On November 8, 2017 and at times prior thereto employees were elevated from a scaffold platform that was supported by a rough terrain forklift.  A guardrail system or other fall protection system was not in place.  The employees were exposed to an approximate 18 foot fall to the ground below. The requirements applicable to construction work under this paragraph are identical to those set forth in 29 CFR 1910.178(l)    29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator has successfully completed the training required by this paragraph (l):    a) Jobsite: On November 8, 2017 and at times prior thereto employees had not been provided with powered industrial truck (forklift) training from the employer to ensure the employees were familiar with truck related topics, workplace related topics, and safe operations.
