In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-16/20 Lathe, #CVTL1 without a cover over a floor opening in the lathe. b. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-16/20 Lathe, #CVTL2 without a cover over a floor opening in the lathe. c. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-20/25 Lathe, #CVTL3 without a cover over a floor opening in the lathe. d. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-25/30 Lathe, #CVTL4 without a cover over a floor opening in the lathe. e.
