29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:    On or about November 28, 2017, and times prior thereto:    a. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-16/20 Lathe, #CVTL1 without a cover over a floor opening in the lathe.     b. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-16/20 Lathe, #CVTL2 without a cover over a floor opening in the lathe.     c. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-20/25 Lathe, #CVTL3 without a cover over a floor opening in the lathe.     d. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK VTC-25/30 Lathe, #CVTL4 without a cover over a floor opening in the lathe.     e. In the Finish Machining Dept., where employees were exposed to an approximately 2-foot trip and fall hazard when removing metal shavings in a HNK KDM-12/13 Lathe, #CVTL5 without a cover over a floor opening in the lathe.
