29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    Jobsite at 587 Main Street, Falmouth, MA:  Construction debris was cluttering the passageways and working areas creating slip, trip, and fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  Jobsite, Second and Third floors:  Employees were engaged in the framing of a new construction multi-use building where window openings were not protected from falling by guardrail systems, safety net system, or personal fall arrest systems exposing employees to falls of greater than twelve (12) feet to the ground below;    (b) Jobsite, Third Floor Roof:  Employees were engaged in installing plywood on the roof without fall protection exposing them to falls of greater than twenty-one (21) feet to the ground below.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:      (a) South Elevation Outside:  A stairway leading from the ground to the second floor was not eqiuipped with a starrail system exposing employees to falls;    (b) Inside First Floor to Second Floor:  A stairway leading from the first floor to the second floor was not equipped with a stairrail system exposing employees to falls;    (c) Inside Second Floor to Third Floor:  A stairway leading from the second floor to the third floor was not equipped with a stairrail system exposing employees to falls.
