29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition: (a) VAR FURNACE AREA, STUB WELDER - The employer did not ensure that the accumulation of titanium dust was removed from the bottom of the stub welder chamber. A flash fire occurred when the titanium electrode fell inside of the welding chamber, disturbing the dust which ignited. The stub welder operator sustained first and second degree burn injuries, on or about November 30, 2017. In the Alternative 29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: (a) VAR FURNACE AREA, STUB WELDER - The employer did not ensure that the accumulation of titanium dust was removed from the bottom of the stub welder chamber. A flash fire occurred when the titanium electrode fell inside of the welding chamber, disturbing the dust which ignited. The stub welder operator sustained first and second degree burn injuries, on or about November 30, 2017. Abatement certification required within 10 days after abatement date. 29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any holes that were 4 feet (1.2 m) or more above a lower level: (a) VAR FURNACE AREA, STUB WELDER - On or about November 31, 2018, and at times prior, employees operated the stub welder, and the employees were exposed to fall hazards that ranged from approximately 10 to 21 feet to the bottom area of the stub welder chamber. Employees were exposed to this fall hazard while performing their regular duties, which included loading and unloading titanium electrodes to and from the chamber, and also cleaning the sides of the chamber from above. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. --------------------------------------------------------------------------------------------------------------------- ISA held on 4/13/18 - changed date of November 31, 2018 to November 30, 2017 in the AVD
