29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: (a) Luis Zamora Roofing at 4122 Vallejo St. Denver, CO 80211: On or before 12/1/17 the exposing, creating and controlling employer did not protect employees from injury in that eye protection was not provided or required when operating pneumatic nail guns while shingling a single family home. This condition exposed employees to eye injuries resulting form flying particles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Luis Zamora Roofing at 4122 Vallejo St. Denver, Co 80211: On or before 12/1/2017 the exposing employer did not protect employees from injury in that fall protection was not worn by roofers engaged in shingling a single family home with a 9/12 roof pitch with an eave of approximately 10 feet. This condition exposed employees to a fall hazard of 10 feet.
