29 CFR 1910.179(n)(4)(i): The upper limit switch of each hoist was not tried out under no load, at the beginning of each operator's shift: (a) Production - On December 1, 2017, the employer did not ensure that the Cleveland Tramrail 3 ton crane's upper hoist limit switch was tried out at the beginning of each operator's shift. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

The monthly inspection shall include a certification record which identifies the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier of the hook being inspected. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
