29 CFR 1926.501(b)(13):  Each employer engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, or personal fall arrest systems:       The employer is failing to ensure employees are protected from fall hazards when conducting installation of shingles as part of re-roofing operations.  Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  The workers were exposed to falls of over 11 feet to concrete below.  This was most recently documented at 211 Ponderosa Circle, Grand Island, Nebraska on or about December 1st, 2017 when the employer had employees working on the roof of this residence without any form of protective system.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:          The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a residential style roof. Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  The workers were exposed to falls of over 11 feet to dirt and concrete below.  This was most recently documented at 211 Ponderosa Circle, Grand Island, Nebraska on or about December 1st, 2017 when employees were observed performing roofing operations at elevation without fall protection training.

29 CFR 1926.1053(b)(1):  Were portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       The employer is failing to ensure employees are protected from fall hazards while utilizing portable ladders as part of re-roofing operations.  Workers were observed climbing up a portable extension ladder which did not extend three feet above the upper surface being accessed by the employees on this site.  The workers were exposed to falls of over 10 feet to dirt and concrete below.  This was most recently documented at 211 Ponderosa Circle, Grand Island, Nebraska on or about December 1st, 2017 when the employer had employees working on the roof of this residence with the above listed issues present as they were going up and down the ladder on site.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:     The employer is failing to provide employees with adequate and effective ladder safety training prior to allowing them to work on a residential style roof.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  Workers were observed climbing up portable extension ladders which were set up incorrectly.  Had the employees had this training the following observed instances would not have been present on the job site.  The ladder did not stand three feet above the upper landing surface (roof) of the building.  The workers were exposed to falls of over 11 feet to dirt and concrete below.  This was most recently documented at 211 Ponderosa Circle, Grand Island, Nebraska on or about December 1st, 2017 when the employer had employees working on the roof of this residence with the above listed issues present as they were going up and down the ladder on site.
