29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable. (a) Roof in front of the building - On or about November 28, 2017 employees were exposed to a flying object hazard while using nail guns and circular saw. These employees were not wearing eye protection. The employer did not enforce the wear safety glasses on this job-site.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. (a) Roof in front of the building - On or about November 28, 2017 employees were exposed to fall, struck-by, and crushing hazards while riding on an unauthorized platform used as a scaffold. This pallet was elevated 16 feet 9 inches above the ground by a Caterpillar TL 1055D Forklift serial no. JR5929. This pallet was not designed by the manufacturer to be used as a platform to transport employees.

(a) Roof in front of the building - On or about November 28, 2017 employees were exposed to fall, struck-by, and crushing hazards while riding on an unauthorized platform used as a scaffold. This pallet was elevated 16 feet 9 inches above the ground by a Caterpillar TL 1055D Forklift serial no. JR5929. This pallet was not secured to the forklift.
