29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more was not protected from falling by guardrail systems, safety nets: (a) Crossroads Worship Center, 190 Lambs Road, Sewell NJ: Employees were observed removing roofing shingles from the roof of the church without the use of fall protection, approximately 36 feet above ground, observed on 12/1/2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Crossroads Worship Center, 190 Lambs Road, Sewell NJ: An employee utilized a Werner aluminum extension ladder to access the roof. The ladder did not extend at least 3 feet beyond the point of access, occurred on 12/1/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
