(a) Dynamic Roofing and Construction, at 5555 N Franklin Street, Denver, CO: On and preceding 11/29/17, the controlling employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were conducted by a competent person.  4 Seasons Roofing and Sheet Metal employees removed and replaced damaged metal decking on the roof of a structure. An employee stepped into a roof opening and fell approximately 23 feet to the ground. The roof opening was not identified in a regular job site inspection.  This condition exposed employees to a fall hazard.
